Membership Requirements & Application Forms

Toledo/Lucas County CareNet is not health coverage. CareNet is a non-profit partnership that helps low-income Lucas County residents get care through its charity care network. To enroll in CareNet’s charity care network, members must be Lucas County residents for at least six months and cooperate with the application process.

Additionally, CareNet members must be ineligible for government healthcare assistance and no access to affordable private coverage. They also must have a household income up to 200% of the Federal Poverty Guidelines:

    • Household of 1 up to $30,120
    • Household of 2 up to $40,880
    • Household of 3 up to $51,640
    • Household of 4 up to $62,400
    • Household of 5 up to $73,160

For Lucas County residents who already have family physicians, please contact their office to see if they accept CareNet. If they do, please contact CareNet at 419-820-0161 for instruction on how to complete the process.

If you do not have a family physician, or your family physician is unable to see you as a CareNet member, please choose a primary care site from the application below that best meets your needs. Call that clinic and ask to schedule a new patient appointment. You will need to take a completed application along with a photo ID and income verification with you to your new patient appointment.
